Rachel is amazing! She listens and really has an amazing handle on our girl stuff! Beyond the normal…. I have been flight crew for 35 years, in my early 60’s and she has helped me with irregular circadian rhythm flying!...

You haven’t lost your edge. Your physiology changed, and nobody told you.
You’re doing everything you’ve always done. The discipline is still there. The drive is still there. But the results aren’t. Somewhere along the way, the sharpness, the energy, and the version of yourself you’ve always relied on started slipping.
This isn’t burnout. It isn’t aging gracefully. It’s a physiologic shift that conventional medicine rarely explains and almost never addresses with any precision.
There is a reason this is happening. And there is a structured, medically grounded path back.
Rachel Smith, DNP brings 20 years of clinical expertise in women’s health, hormone optimization, and metabolic medicine to one focused mission: helping high-achieving women understand exactly what is happening in their body and build a precise plan to restore their clarity, energy, and performance.
Applications are reviewed personally. Not everyone is the right fit, and that’s by design.
Hormonal & Metabolic Restoration
Your hormones, metabolism, and energy systems are interconnected. When they shift, everything shifts. We identify exactly where the disruption is and build a precise plan to restore balance.
Cognitive Clarity & Performance
Brain fog, slowed thinking, and diminished drive are physiologic, not personal failings. We address the underlying systems so your mind performs the way you expect it to.
Body Composition & Strength
This isn’t about a number on a scale. It’s about preserving the lean mass, metabolic flexibility, and physical capacity that keep you performing at the highest level for decades.
Longevity & Cardiometabolic Health
What you do now determines what your body can do at 70, 80, and beyond. We use advanced risk assessment and precision data to protect your long-term capacity, not just manage today’s symptoms.

One Woman’s Inspiring Transformation
How This Practice Works
Most healthcare encounters are transactional — a visit, a prescription, a follow-up. What you will experience here is fundamentally different. This is a longitudinal clinical partnership, designed to evolve with your physiology over time and produce measurable, meaningful change. Every client begins with the Precision Performance Assessment. From there, Rachel personally confirms the right program fit, your Precision Performance Blueprint is developed, and the work begins — data-informed, clinically guided, and built specifically for your physiology.

So my name is Rachel, and I'm from Sarasota, Florida. And two and a half years ago, I started on a journey that has truly transformed my entire life. And that journey is really focused on putting myself at the top of the list, making myself a priority, focusing on full body health, both inside and out. This journey started during the pandemic when I shot up over a hundred pounds. I was unhealthy. I was choosing unhealthy foods because of the work that I was in. I was working seven days a week, and I was really unable to choose a healthy lifestyle. The gym was closed. My trainer was unable to continue with his business. One day, went to an office to see my dentist and learned that I had won a basket for referrals. They asked to take a picture of me. Before I knew it, the picture was posted on Facebook, and I looked at the picture and did not recognize the person that I had become. I was nearly 260 pounds. I was pre-diabetic. Every joint in my body hurt when I got out of bed, and I just was absolutely at the end of my rope. I kept saying to myself, you're too young to feel this bad. And so I decided that it was time for me to truly take control of my life and my health. As we all know, we're only given one body and one life to live, and I didn't want the rest of my life to be miserable and to be uncomfortable and certainly unhealthy. This journey has really been a lot for me to relearn about making sure that I'm focused on activity, that I'm focused on looking at macros and looking at food as a source of energy and not comfort. So I've had to really relearn that food is to nourish your body, to make you healthy, to give you energy, to make you feel good, and not to make you feel like that it's something that you should be doing because you're happy or you're sad or you're lonely or you're anxious. This journey has been one where I've done a lot of self-discovery. I remember the first day I went into the gym at nearly 260 pounds. How am I going to do this? I felt so uncomfortable and so self-conscious. I looked around, I saw all these healthy, young individuals. Every mirror that was around me, I tried not to look at myself. It's been a slow progression of going from 30 minutes a day to just cardio to basically two and a half hours a day where I'm spending 40 minutes on cardio and nearly 90 minutes on lifting heavy weights. I'm actually in better shape now than I was in my 30s when I was running full marathons. I lost my only sibling. She was 44 years old. She was morbidly obese, hypertension, diabetes, everyone in my family, the diabetic, except for me. And I just didn't want to spend the rest of my life on a ton of medications and being unhealthy. You know, I've got the best years of my life coming up and why should I spend them feeling horrible? And I want to be able to keep up with grandchildren and be able to travel all over the world and walk everywhere and be active and not just sit around and be miserable. Because we're all different, we're all unique, it took a great deal of really understanding where my fuel was coming from and understanding what fuel I needed in terms of back rows from certain foods and not just looking at calories and not just looking at, oh, go take, you know, a medication to help you lose weight. I mean, it truly is a comprehensive system of things that individuals need that are not the same. There's not a one size fits all for everyone. And so for me, it's been really figuring out what it takes, what's that recipe, right? Or what do I need in my tool belt in order for me to maintain or to change, you know, the way I look and feel.

Hormones, metabolism, sleep, stress physiology, and body composition are interconnected. We look at the full picture — not individual markers in isolation.

LONGITUDINAL ITERATION
Care evolves over time using data, clinical response, and refinement. This is not a protocol applied to everyone. It is a strategy built for you.

Frequently Asked Questions
Who is this practice for?
This practice is designed for high-achieving women in midlife who are noticing a meaningful decline in their clarity, energy, resilience, and performance, and want a precise, medically grounded explanation and plan. If you are looking for a quick fix, a prescription without a strategy, or a general wellness program, this is not the right fit. If you are ready to understand exactly what is happening physiologically and commit to a structured path forward, you are in the right place.
How much time does this require each week?
The program is designed to work within the life of a high-performing woman, not compete with it. Medical visits are typically monthly or as clinically indicated. Coaching and check-ins are structured and efficient. The foundational work of nutrition, movement, sleep, and stress regulation is built into your existing life, not layered on top of it as another obligation. The investment is in the outcome, not in the hours.
Is care available virtually?
Yes. The Precision Performance Assessment and ongoing program care are available via telehealth for clients in Florida, Iowa, Massachusetts, Arizona, Colorado, and Alaska. In-person visits are available at our Bradenton, Florida location.
How long is the program?
The Precision Performance Partnership is a six-month engagement. The Precision Performance & Longevity Partnership extends to twelve months for clients pursuing deeper physiologic refinement and long-term risk optimization. Both are structured as longitudinal partnerships rather than a series of disconnected visits, because meaningful physiologic change requires time, data, and iteration.
Do you offer hormone replacement therapy?
Yes. Hormone evaluation and replacement therapy for women, including estrogen, progesterone, and testosterone where clinically appropriate, is a core component of the practice. Men’s testosterone replacement therapy is also available. All hormonal care is individualized, data-informed, and integrated into a broader metabolic and performance strategy.
What if I’m not sure I’m ready to commit to a full program?
The Precision Performance Assessment exists precisely for this moment. It is a standalone clinical evaluation, not a sales conversation, and it gives you a clear picture of what is happening in your body and what a structured path forward looks like. Many women find that the assessment itself provides more clarity than years of fragmented medical appointments. From there, the decision about next steps is entirely yours.
Do you work with weight loss medications like GLP-1s?
When clinically appropriate, yes. Medications including GLP-1 receptor agonists are available as one tool within a comprehensive metabolic strategy. They are never the entire plan. The focus is always on understanding the root physiology, building a strong metabolic foundation, and using medications strategically, not as a substitute for the deeper work.
